    Please Note:  One badge & lanyard are provided each year. A replacement fee for lost or damaged School District property will be assessed.  Examples:  library or text books, iPad, uniforms, etc.

    There is an individual maximum of $40 (or 2 activities) and a family maximum of $120 (or 6 activities) per year for intermediate schools. There is an individual maximum of $50 (or 2 activities) and a family maximum of $150 (or 6 activities) per year for high school.

  • IMPORTANT:  Families need to complete a Household Income Form in order to receive a student fee waiver. Fee waivers are for current year general, athletic and activity fees only. The information on the Household Income Form will determine if your student's fees can be waived.
     
     
    Each household in the District will receive a Household Income Form (one form per household). Please complete this form and return back your child's school or to the Kolak Education Center, Business Office, 1500 Fourth Street, Beloit, WI 53511.
